- Why things fail
- How can we design for failure-resistance.
Some engineering failure examples that are more widely known include:
- Titanic hitting an iceberg
- Spaceshuttle Challenger explosion
- The Tacoma Narrows Bridge collapsing
- A bone fracture
- Aloha Airline 243 - a section of the fuselage flew off in mid-air because of metal fatigue (image below). No passenger died. The pilots were superb in landing it safely. Here is one Youtube.
