Graduation Application Requirements

Deadlines:

Submit all forms & documentation to the Engineering Office, SCI 163 |

Check the status of your application as posted on your Program Head's office door (CE SCI-168A, EE & CmpE SCI-170, ME SCI-100, and SCI-163 for GE status). |

Approved applications will be ready to be picked up from the Engineering Office |

Applications must be submitted to the University for final approval. Bring them to the Student Services building One-Stop desk. |
The following forms must be completed. These forms are available on-line (see below for instructions). If you choose to complete hard-copy forms instead, remember that they must be typed. The University Graduation Application Form can be picked up in at the One-Stop desk in the Student Services building. You must pick up your Student Planning Worksheet from the Engineering Office.

Please note the following details:
- Two sets of documentation are required. One set is attached to the University Graduation Application and the second set is attached to the General Education Graduation Application form.
- List only upper division (300 and above) engineering courses on in the University Graduation application "Major Courses" section, with the following exception (exception for Electrical Engineering students only): If you took ENGR 201 instead of 303, enter 201 on this list Also list any lower division electives that you have taken for your major requirements.
- When completing the University Graduation Application form, do not fill in an "Emphasis" after the "Major."
- Do not enter your advisor’s name on the University Graduation Application form; enter the name of the Program Head for your major. They are as follows:

- You must pick up your approved application from the School of Engineering and deliver it to the One-Stop desk in the Student Services building. You have not completed your application process until it has been submitted to the University and the application fee has been paid. The School of Engineering is not responsible for approved applications that are not picked up and properly filed with the University.
IMPORTANT GENERAL EDUCATION NOTE: Only students following the Engineering G.E. program are required to file a General Education Graduation application form. However, because we are required by our accrediting agency (Accreditation Board of Engineering and Technology, ABET) to document all of our students’ General Education programs, we ask you to please complete the Academic Progress Record normally found in the printed University Class Schedule, if you are following the University G.E. program. Please print neatly, enter your name and student number at the top of the form and turn it in with your graduation application. No documentation will be required. The Engineering office can supply this form to you, if necessary. |
ON-LINE GRADUATION APPLICATION PROCEDURE
All graduation applications (1 form for students following the University G.E., 2 forms for students following the Engineering G.E.) and instructions are available on-line through the University web page. Location is http://www.sfsu.edu/~admisrec/forms/formstoc.htm Then type in "student forms" and go from there.
The B.S. Graduation application online includes both the Major application form and the Engineering G.E. application.
Note that the Univ. Grad. Appl. has two parts to complete, the Cover page and the Major page. Complete both parts and print out your form. Attach all of the documentation needed for this form that is listed on the other side of this paper. Also attach the Student Planning Worksheet that you have picked up from the Engineering office.
In addition, the G.E. Appl. also has two parts, the Cover page, and the course list page. Complete both the cover page and the course list page. Print out your form. Attach all of the documentation needed for this form that is listed on the other side of this paper.
Submit all completed forms and your Student Planning Worksheet along with documentation to the Engineering office, SCI 163. We will briefly review your paperwork for obvious omissions, and forward it the appropriate faculty member for evaluation and approval signatures.
